Request for Application

The North Carolina Triad Affiliate of Susan G. Komen for the Cure® is pleased to announce that the 2012-2013 Request for Application (RFA) is now open.  The Affiliate is accepting Applications for non-duplicative, community-based breast health education, breast cancer screening and treatment support programs for the medically underserved in our eleven (11) county service area comprised of Alamance, Davidson, Davie, Forsyth, Guilford, Randolph, Rockingham, Stokes, Surry, Wilkes and Yadkin counties in North Carolina.

• Applications are accepted from US non-profit organizations, governmental agencies, educational institutions and Indian tribes in the service area.
• The Application deadline is November 4, 2011, at 4:00 p.m.
• The grant cycle begins on April 1, 2012, and ends March 31, 2013.

The RFA includes the instructions for applying for a Komen NC Triad Affiliate Community Grant.  The guidelines must be met in order for the proposal to be reviewed.
